Dear parents,
The annual Health Screening for our pupils will be held between 1/7/13 (Monday) and 11/7/13 (Thursday).
For P1
P1 Pupils will be taking the vaccination so consent forms will be given.
Please ensure that the parents fill up all sections of the the form and sign on the appropriate space. Only a black or blue pen should be used to fill up the forms. Without parental consent, the nurse will not be able to carry out the vaccination for the child.
